Elcies Disability Care is a new venture for Lutheran Care after being approved as a registered Disability provider with the NDIS. This new disability services arm will support people with a disability to participate in the activities they choose and achieve the goals they set for themselves.
We are looking for an experienced Support Coordinator to join us at this exciting grass roots stage and help shape the role as we grow.
In this role, you will be empowering our clients by advocating for their choice in control for the services they chose by ensuring you understand their goals and their required supports within their NDIS plans. You will bring with you a commitment to collaboration, empowerment and capacity building, providing high quality, collaborative support coordination and capacity building for clients, carers and families.
This position is perfect for a Support Coordinator who wants to make their mark in an organisation, bring enthusiasm to develop systems which work for you and the client, as well as commitment to best outcomes for clients.
A current drivers licence and your own vehicle is essential for this position. A Tertiary qualification within health/disability or equivalent practical experience is required.
